• Any investigations performed in the last year (bloodwork, ECG, imaging).

• Relevant physician consultation notes (both medical and psychiatric).

• Hospitalization consult and discharge notes.

Please Note: Occasionally we may recommend that a patient be referred to a higher level of care at a hospital-based Eating Disorder Program. You will be provided with a summary of our assessment and recommendations, and in those cases it will be the primary care provider/referring physician's responsibility to submit the necessary referral.
